Procuring Agency Referral Report

The Procuring agency referral report: Ethical Supplier Mandate provides procuring agencies with a template of the information required when referring an alleged breach of the Mandate to the QGP Compliance Branch.

The Procuring agency referral report: Ethical Supplier Threshold provides procuring agencies with a template of the information required when referring an alleged breach of the Threshold to the QGP Compliance Branch.

Example Report

The Compliance Branch have prepared two example reports for breaches under both the Mandate, specifically in relation to the BPPs and the Training Policy. We encourage you to download these example reports to use as a guide for the level of detailed required in your own report when you are referring an investigation to the Compliance Branch.

The sample reports relate to a fictitious supplier/project but has been extrapolated from real examples of non-compliance.

Proposed Sanction Notice templates

The Mandate and the Threshold Proposed Sanction Notice templates provide procuring agencies with a guide to issuing suppliers in breach of the Mandate and Threshold with a proposed sanction.

Outcome Notice templates

Once a determination has been reached, for example demerits or a sanction has been applied to a supplier or it was determined that no penalties are being applied to a supplier, the appropriate Outcome Notice will be issued to the supplier informing the supplier of the outcome.
